1. Each parent must complete the Online Member Registration completely which is available under the submenu on the left side. 
2. The following documents must be completed in entirety, signed, and either mailed to P.O. Box 170417, Arlington TX 76003 or scanned and emailed to info@texasexpress.org. 

• Copies of 2012-2013 report card and a copy of birth record
• Commitment Form
• Player Code of Conduct  (Players & parents read carefully)
• Parent Code of Conduct (Parents read carefully)
• Media Consent Form 
• Risk and liability Form

    3.    Make your payment/s on line or mail to the p.o. box above.  I understand that alI fees are non-refundable.  If I do not sign the refund policy,  I understand that refunds will not be issued regardless of whether or not I sign the commitment document and refund policy.  Uniform refunds will not be issued once ordered with number personalization.  All other non-tangible items are non-refundable as described in our refund policy.
